At HealthBridge, we are proud to offer a full continuum of care for your loved ones, with multiple levels of service, including: high-acuity complex medical, acute rehabilitation, subacute care and outpatient rehabilitation services.

Our dedicated team of experienced healthcare experts will be available 24/7 in our patient- and family-friendly facilities. Whether the patient is joining the care collaborative journey at the trauma or acute level, the in-patient rehabilitation level, or is in the process of returning home — higher functioning, but still in need of improving basic skills to regain independence, we are prepared to help our patients at every stage of recovery.
